For your convenience, I have put these forms online.  If you choose, please print these forms out and bring the completed Intake Form and Disclosure Agreement to your first session. 

If you will be doing therapy by phone, please complete the Intake Form and the Disclosure Agreement and mail them to me at the address on the Disclosure Agreement.  Be sure to include  a check for your first session.  I must receive this information prior to the first session.
